Forms & Data Entry

The forms grouped under this tab are used for data input, entry and collection by our volunteers and interns. By utilizing these through interactions with students, the College Success Centers are able to paint an accurate picture of how many students are utilizing the centers, and the frequency with which students need assistance with different components of the college choice process. These forms are password protected and utilized within the centers by our staff with students.

CSC Activity Form
This form is the most commonly utilized as center staff will log the student visits in addition to what they helped the student work on during the visit to the center.

College Knowledge Pre-Test
This form is completed by each student once before beginning the process of underoing formal college searches, financial aid applications and other components of the college process. This test allows students to demonstrate what knowledge they have of college applications and processes before they receieve assistance from the college success centers.

College Knowledge Post-Test
PLEASE NOTE: This form will be initially unavailable so as to make sure students are completing the pre-test first and not mixing the forms up.

This form is completed by each student once after the process of underoing formal college searches, financial aid applications and other components of the college process. This test allows students to demonstrate what knowledge they have of college applications and processes after they receieve assistance from the college success centers. This test is the SAME as the pre-test and should demonstrate how and in what areas students have grown in their understanding of the college application process.

College Fair/Visit Form
This form is utilized during organized college fairs and visits where groups of students and/or grade levels are able to visit a fair or visit a college or university. This allows center staff to gain an understanding of how many students have been able to benefit from exposure outside of their respective college success center.
